These forms are to be used by Oklahoma Juvenile Court Staff.
Word Version: You can download the Microsoft Word documents and save the document.
WordPerfect Version: You can download the WordPerfect documents and save the document.
Excel Version: You can download the Excel Workbook and save
the workbook.
The Workbook contains worksheets for all Deprived Juvenile Uniform Orders; an
Instruction Page and a Case Information Page.
To fill out a form, first select the YELLOW tab titled Case Information to enter the party information. Using the drop down select the Party Type; enter the Party Name; Party DOB; Attorney for Party on Same Row. Using the Drop Down select a Case Type; enter the Case Number. Using the drop down select the County. Use the drop down to select an Additional Case Type; enter the Additional Case Number. All Uniform Orders will be populated with this data. To re-use the forms with the pre-entered data save the Excel Workbook in its own case file.
On each form the YELLOW fields are available for data entry. When the data is larger than the field shown, word wrap is available and the row will need to be expanded to correctly show the data and print. Place the curser in the far left column with the row numbers and drag downwards on the lower line of the row, dragging far enough to show the data text.
All Documents Currently Require Microsoft Word, Excel, or WordPerfect.
